I can't give yo exact ingredients because I don't use measurements when making my sausage. I shake in what looks right, mix well, break off a little piece, fry, taste, and adjust the seasonings until it's just right. But I cna get you started and with approximate measurements.

Breakfast Maple Pork Sausage

Ingredients:
1 lb. ground pork (grind my own from pork butt)
1 1/2 tsp. Kosher Salt
2 tbs. rubbed sage
1 tbs. ground Thyme
2 tps. ground, black pepper
1/4 tsp. cayenne pepper
2 tbs. Grade B Maple Syrup (or 2 tbs. brown sugar)
1 tbs. granulated onion powder
1 tbs. granulated garlic powder
1 large egg (reduces shrinkage while cooking)

Mix all ingredients together and break off a small piece. Fry in a hot skillet. Adjust the seasonings.

Tip; you can replace the syrup with apple sauce, or apple juice if you want.

To make this into a pizza topping sausage, add 1/2 tsp. Rosemary, and 1/2 tsp. fennel, and 1/8 tsp. mesquite flavored liquid smoke.

To change into a sausage for pork sandwich, add 1 tsp. cinnamon, 1/2 tsp. turmerick, 1/4 tsp. allspice, a dash of nutmeg, and 2 tbs. chili powder.

Replace 1/2 of the sage with 1 tsp. each oregano and sweet basil and mix with an equal amount of ground beef for meatballs, adding diced onion and diced green peppers of course.

Hope this gets you into making your own basic pork sausage. You can really play with this stuff. It's versatile and yummy.
